Scheduled Weekly Hours:

Emplify Health by Gundersen Health System is seeking a full-time Social Services Liaison. This is your opportunity to be a part of a great organization who priorities patient care!

What you will do:

• Work 40 hours/week

• Monday-Friday, no weekends or holidays

• Work remotely *Must live in WI, MN or IA*

• Partner on special projects and duties to assist our patients and their discharge needs.

What you will need:

• Bachelor's degree in Social Work or in a related field

• 1 year of previous experience in human services or health care
