- Key Responsibilities
- Oversee daily patient care coordination and remote support services
- Supervise, mentor, and support patient care coordinators and support staff
- Ensure timely patient follow-ups, care updates, and service assistance
- Monitor patient cases, escalations, and service quality standards
- Collaborate with healthcare providers to align care plans and patient needs
- Maintain accurate patient records and ensure HIPAA compliance
- Analyze patient care metrics and implement process improvements
- Train staff on patient communication best practices and care protocols
- Address complex patient concerns with professionalism and empathy
- Qualifications
- Bachelor’s degree in Healthcare Administration, Nursing, Public Health, or a related field
- Minimum of 1 year of experience in patient care coordination, healthcare operations, or case management
- Prior leadership or team management experience
- Strong understanding of healthcare workflows and patient service standards
- Excellent communication, organizational, and problem-solving skills
- Experience with EMR/EHR systems and remote care tools
- Ability to manage multiple patient cases while maintaining high-quality service
- Preferred Qualifications
- Nursing background (RN/LPN) or clinical experience
- Experience in telehealth or remote healthcare environments
