As a Home Health Physical Therapist, you’ll practice at the top of your license—bringing skilled, one-on-one care directly into patients’ homes where it matters most. You’ll independently evaluate patients, design personalized, evidence-based treatment plans, and help restore mobility, reduce pain, and improve daily function and independence.
You’ll also lead and support Physical Therapist Assistants (PTAs), ensuring that care is delivered safely, effectively, and aligned with each patient’s goals. Working closely with an interdisciplinary team, you’ll continuously adjust treatment plans to drive the best possible outcomes.

Requirements
Successful completion of accredited physical therapy program and state licensure exam.
Valid Physical Therapy license
1 year of home health experience preferred
CPR certification
Valid Driver’s license
Valid Auto Insurance
HomeCare HomeBase experience preferred

Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
