Position Overview

Key Responsibilities
As a Patient Care Coordinator on our Refill Team, you will be responsible for a variety of tasks that are essential to providing excellent patient care. Your daily responsibilities will include:
- Answering Incoming Calls: Handle incoming calls from patients, caregivers, branches, and patient reference sources regarding nutrition therapy request status. You will be the first point of contact for patients seeking information about their nutrition treatment, so excellent communication skills are essential.
- Call Triage: Effectively triage calls to other departments as needed, ensuring that patients are directed to the right resources for their specific needs. This requires strong organizational skills and the ability to make quick, accurate decisions.
- Patient Demographics Verification: Verify patient demographics and insurance information to ensure accurate shipment of nutritional products. Attention to detail is crucial in this aspect of the role to prevent any delays or errors in patient deliveries.

Required Qualifications
