**Key Responsibilities**
As a Customer Service Representative, you'll be responsible for:
* Providing after-hours support to triage urgent clinical needs of patients, caregivers, and referral sources
* Assisting local Patient Registration teams with new patient referrals and ensuring accurate information collection
* Meeting standardized performance metrics for calls and referrals
* Communicating coordination of care information with team members, patients/caregivers, referral sources, and home health agencies
* Delivering quality customer service during all interactions and maintaining organized file systems for document tracking and patient management
**Requirements**
To be successful in this role, you'll need:
* High school graduate or GED required
* At least one year of previous call center experience
* Proficiency with Microsoft Office Suite and keyboarding skills
* Strong interpersonal and communication skills
* Ability to promote a compassionate, caring, customer service-oriented culture
* Knowledge of computerized information systems
* Dedicated workspace that is quiet and free of distractions
* Ability to maintain confidentiality of patient information
**Preferred Qualifications**
While not required, we prefer candidates with:
* Associate's or Bachelor's Degree
